Our Process from Admission to Discharge
Depending on your needs, your visit can be lengthy to ensure that you get the care you need.
1. Referring Specialist completes an LOM
2. Our admissions team, receives the LOM and requests authorization from the funders
3. Authorization received and communication with the referring specialist and facility are done
4. Transfer confirmed
5. Patient Admitted
6. initial Assessment conducted by Nurses and Doctor
7. Therapists conduct their initial assessment and a treatment plan is developed
8. Treatment followed-through and discharge planning commenced
9. Patient discharged, provided with a DC Report and a follow-up appointent made with referring specialist
We treat a range of conditions that vary in severity. This includes complex cases such as neurological trauma, stroke rehabilitation, and orthopaedic injuries that may threaten limb function or require urgent intervention. Additionally, we care for patients with conditions that, while not immediately life-threatening, could worsen without timely medical attention, such as post-surgical recovery or moderate trauma. Some patients may require long-term admission and specialized rehabilitation, while others may benefit from short-term care or outpatient services, depending on the severity and nature of their condition.
